Not possible from a phonocardiogram.

That is the only possible way to do it, since HRV requires a normal QRS complex preceded by a normal P-wave to identify a ventricular depolarization. It is only possible to estimate HRV from the EKG.

No.

The heart rate is controlled by a number of influences, from the Herring-Breuer reflex to preload and others. The heart rate (in a healthy heart) is determined by sinoatrial (S-A) node activity that triggers atrial contraction (P-wave) and then after a short delay in the atrioventricular (A-V) node, ventricular contraction (QRS-complex). So a normal QRS-complex preceded by a normal P-wave defines a normal heartbeat. It is impossible to determine a normal heartbeat from ectopic rhythms (extra beats originating other than in the S-A node) using only a peripheral indication of ventricular activity, such as a phonocardiogram, digital pulse plethysmography (pulse oximetry), or anything else other than an EKG.
