how do we know the size of an obstacle in an image?

See my spatial calibration demo, attached. It's meant for flat scenes where every place in the scene is the same distance from the camera. For your images though you're going to have to calibrate your camera so that the top of the screen has more cm per pixel than the bottom of the image.
